1095-B - How do I complete the covered individuals' section?

Any individuals that were covered for at least one month should be listed in the covered individuals' section (green column headers). This includes employees, spouses, and dependents who were enrolled in the offered coverage.



The Covered Individual's data should start on the same line as the employee data. There are two options for completing this:


  • Option 1 — Highlighted orange above: You can add blank rows to ensure that you aren't listing the CIs on the wrong employee line.
  • Option 2 — Highlighted teal above: You can duplicate the employee data to ensure that you aren't listing the CIs on the wrong employee line.

Both options listed above are correct and will import without issue, so use whichever you prefer and is easier for you!
