How do I download the header form? (1094-C, 1094-B, 1096, W3)

Follow the steps below to download your header form.

Click on your filing to highlight it, then select Download.

Choose the summary form option (right button) to download only the summary form. .

This will download as a zip file to your downloads folder of your computer.
