How do I download the header form? (1094-C, 1094-B, 1096, W3)

Login to your BoomTax account and navigate to your filing.

Click "Download" and "To PDF."

Choose the summary form option (right button) to download only the summary form. .

This will download as a zip file to your downloads folder of your computer.
