1099 Import Spreadsheet: Required Fields (Tax Year 2025)

This guide covers the required fields for importing 1099 and W-2G form data into BoomTax. Download the templates here.


Fields Required for All 1099 / Payee Forms

These fields are required regardless of which form type you're filing:

  • Recipient first name (or company name for businesses)
  • Recipient last name (leave blank for businesses)
  • TIN type: Must be SSN, EIN, ITIN, ATIN, or NA
  • TIN: Social Security Number or Employer Identification Number (with or without dashes)
  • Street address
  • City or town
  • State
  • ZIP code
  • Payer contact phone number: Minimum 10 digits

1099-NEC (Nonemployee Compensation)

For reporting payments of $600+ to contractors, freelancers, and other nonemployees.

  • 1. Nonemployee compensation: Dollar amount paid (required)
  • 2. Payer made direct sales: Yes/No (required)
  • 10. 2nd TIN notice: Yes/No (required)
  • 4. Federal income tax withheld (if applicable)

Note: Federal tax withheld cannot exceed 24% of compensation.

1099-MISC (Miscellaneous Income)

For rents, royalties, medical payments, attorney payments, and other miscellaneous income.

  • At least one amount box (Boxes 1-12) must have a value
  • 7. Payer made direct sales: Yes/No (required)
  • 13. FATCA filing requirement: Yes/No (required. Use "No" if not applicable, do not leave blank)
  • 10. 2nd TIN notice: Yes/No (required)

Amount boxes include:

  • 1. Rents
  • 2. Royalties
  • 3. Other income
  • 5. Fishing boat proceeds
  • 6. Medical/health care payments
  • 8. Substitute payments
  • 9. Crop insurance proceeds
  • 10. Gross proceeds to attorney
  • 11. Fish purchased for resale
  • 12. Section 409A deferrals

1099-INT (Interest Income)

For reporting interest payments of $10 or more.

  • 1. Interest income: Dollar amount (at minimum)
  • 12. FATCA filing requirement: Yes/No (required)
  • 11. 2nd TIN notice: Yes/No (required)
  • 10. Payer's RTN: Routing transit number (optional, 9 digits, numbers only)

Optional boxes: 2. Early withdrawal penalty, 3. Interest on US Savings Bonds, 4. Federal tax withheld, 5. Investment expenses, 6. Foreign tax paid, 7. Foreign country, 8. Tax-exempt interest, 9. Private activity bond interest.

1099-DIV (Dividends and Distributions)

For reporting dividends of $10 or more.

  • 1a. Total ordinary dividends: Dollar amount (at minimum)
  • 11. FATCA filing requirement: Yes/No (required)
  • 10. 2nd TIN notice: Yes/No (required)

Validation: Box 1b (Qualified dividends) cannot exceed Box 1a. Subcategories of Box 2a (2b, 2c, 2d) must each be less than or equal to Box 2a.

Optional boxes: 1b. Qualified dividends, 2a. Total capital gains, 3. Nondividend distributions, 4. Federal tax withheld, 5. Section 199A dividends, 6. Investment expenses, 7. Foreign tax paid, 9-10. Liquidation distributions, 12-13. Exempt-interest dividends.

1099-R (Distributions from Pensions, Annuities, IRAs, etc.)

For reporting retirement plan distributions of $10 or more.

  • 1. Gross distribution: Dollar amount
  • 2a. Taxable amount
  • 7. Distribution code: Select from approved IRS distribution codes
  • 7. IRA/SEP/SIMPLE: Yes/No

Validation: Capital gain (Box 3) cannot exceed taxable amount (Box 2a). Box 8 percentage must be 0-99.

Optional boxes: 2b. Taxable amount not determined, 4. Federal tax withheld, 5. Employee contributions, 6. Net unrealized appreciation, 8. Other amount/percentage.

1099-K (Payment Card and Third-Party Network Transactions)

For reporting payment card/third-party network transactions.

  • 1a. Gross amount of transactions: Dollar amount
  • 3. Number of payment transactions: Required integer
  • 2nd TIN notice: Yes/No (required)
  • 2. Merchant category code: If provided, must be exactly 4 digits (0001-9999)

Validation: Card not present transactions (Box 1b) cannot exceed gross amount (Box 1a).

Optional: Boxes 5a-5l for monthly transaction breakdowns (January-December), PSE name and phone, filer type, transaction type.

1099-C (Cancellation of Debt)

For reporting cancelled/forgiven debt of $600 or more.

  • 1. Date of identifiable event: Date format (required)
  • 2. Amount of debt discharged: Dollar amount (required)
  • 4. Debt description: Text (required)
  • 5. Was debtor liable? Yes/No (required)
  • 6. Identifiable event code: Select from approved codes (required)

Validation: Interest (Box 3) cannot exceed amount of debt discharged (Box 2).

W-2G (Certain Gambling Winnings)

For reporting gambling winnings of $600+ (at 300x the wager or more).

  • 1. Reportable winnings: Dollar amount (required)
  • 2. Date won: Date format MM/DD/YYYY (required)
  • 3. Type of wager: Select from approved wager types (required)
  • 4. Federal income tax withheld (if applicable)
  • 5. Transaction: Required for wager types keno (4), bingo (5), slot machines (6), and other (7)

Validation: Federal tax withheld cannot equal or exceed reportable winnings.

Optional: 6. Race, 7. Winnings from identical wagers, 8. Cashier, 9. Winner's TIN, 10. Window, 11-12. Identification numbers.

1099-SA (Distributions from HSA, Archer MSA, or MA MSA)

For reporting distributions from health savings accounts.

  • 1. Gross distribution: Dollar amount
  • 3. Distribution code: Select from approved codes
  • 5. HSA, Archer MSA, or MA MSA: Account type indicator

Optional: 2. Earnings on excess contributions, 4. FMV on date of death.


Common Import Mistakes (All Forms)

  • TIN Type must be "SSN" or "EIN": Other values like "ITIN" or numbers will cause an error.
  • SSN leading zeros stripped by Excel: Format the column as Text before entering data.
  • FATCA field left blank: 1099-MISC, 1099-INT, and 1099-DIV require this field. Enter "No" if not applicable.
  • State not abbreviated: Use "TX" not "Texas".
  • Missing column headers: All template columns must be present, even if empty.
  • State-specific payer ID rules:
    • WI: 15 digits, must start with 036
    • UT: 14 digits, must end with WTH, no hyphens
    • MD: 8 digits only
